What is your current contribution to the practice you are involved with? Calculate the revenue you generate. Also, evaluate the overhead in your practice site
One of the most impressive things nursing has to offer is variability and flexibility. No two areas of nursing are run the same. Advance Practice Registered Nurses (APRNs) have evolved over a lengthy period of time and have become increasingly more important in the delivery of health care in the United States (Waxman, 2018). My current contributions to the practice I am involved are to improve patient experience through education, manage acute care situations, and manage prescriptions. My primary role is to assess patients, diagnose conditions, and implement a plan for managing care. Patients are seen for acute conditions. My goal is to efficiently educate the patients so they can effectively manage their care. Informed patients will have better healthcare outcomes.
In the college-based clinic I work in, funding for health services is currently included in tuition. Students are seen free of charge in the clinic, although this may change soon. However, if the patients are sent out for further evaluation, it is at their expense. Managing overhead can be difficult, as there are many supplies that must be kept on hand, even though they may be rarely used. To offset some costs associated with the clinic, the university has established Memorandums of Understanding (MoUs) with outside agencies such as the Mississippi State Department of Health (MSDH). A MoU is an agreement between parties and can be bilateral (two parties) or multilateral (more than two parties), serving as an expression of aligned will between the parties in question and depicts the intent of a common line of action (Memorandum of Understanding (MOU), 2021). In addition to using MoUs, the staff must be careful not to waste materials and medications. Thus, prescription medication for employees is limited. Their medications are called in to their pharmacies. To eliminate excessive staff, student workers are hired to manage non-health related services. Efficient offices that keep support staff busy with the work of the business and are busy with patients when open will have lower overhead rate (Waxman, 2018).
Based on your revenue calculations, what amounts of funds are left for salary and profit? How does this correlate with your current salary?
Funds for salaries are calculated at the end of the fiscal year for the upcoming year, based on current salaries for positions. Annually, those salaries are evaluated. Funds may be moved from one line item to another. For example, if there is an excess in contractual services, it may be moved to commodities or salaries. No profit is indicated, as the university is a “not for profit” organization. Unfortunately, receiving raises has been rare. Recently, a compensation study was completed to ensure that employees were paid competitive rates for each position within the geographical location. Results indicated the need for raises for some employees.
What opportunities exist in your practice that add value to your services and improve patient care quality?
There are several opportunities to enhance the services at the clinic. Attending healthcare conferences and trainings keep the staff abreast of healthcare challenges and changes. Using the information learned in these setting allows the provider to initiate change withing the facility based on the most recent evidence. These opportunities are shared with the student body in interactive and meaningful ways so they can comprehend and utilize the information. Frequent in-house trainings have also been instrumental in the growth of the staff as well as the students. For example, this week, students were introduced to heart healthy information through the “Go Red” campaign. Student health leaders presented literature to the student body and the staff at the infirmary did assessments for heart disease. This made students more aware of issues concerning cardiovascular disease. Continuing education and collaborative programs have been instrumental in improving overall health of the campus.

The types of activities that advanced practice nurses (APRNs) get involved in within a healthcare organization affect financial performance in one way or another. Therefore, as they perform their leadership roles, doctorally-prepared nurses should identify the contributions of APRs to practice and how such contributions are affecting the financial performance of an organization (Waxman, 2018). APRNs are directly involved in patient care in their practice settings. Some of their contributions to practice, similar to what you have explained in your discussion, include providing patient education, managing acute care situations, and managing prescriptions (Salmond & Echevarria, 2017). Patients who are adequately educated about self-care can manage their conditions effectively at home thereby preventing within 30-days readmissions. Additionally, effective management of acute situations enhances recovery and prevents prolonged length of hospital stay. Besides, effective management of prescriptions prevents medication errors and promotes patient safety. Preventing 30-day readmission, prolonged length of hospital stay, and medication errors are associated with improved financial performance in healthcare organizations (Cleveland et al., 2019). As an APRN, you should engage in activities that improve the financial performance of your organization.
Rapid reforms in the healthcare industry present numerous opportunities for APRNs, especially the Doctor of Nursing Practice-prepared nurses, to improve the quality of care to patients. For example, changes in healthcare delivery models and technological integration into healthcare delivery present an opportunity for nurses to receive advanced training in order to learn the knowledge and skills required in those roles (Salmond & Echevarria, 2017). As you have stated, one of the opportunities in your practice setting concerns healthcare conferences and pieces of training that keep the staff abreast of healthcare challenges and changes. It is advisable that you take advantage of these opportunities as the conferences and pieces of training will equip you will the skills and knowledge relevant for today’s nursing practice.
